Earlier this year, the NHS set out its priorities for the next 10 years in its Long Term Plan.

Our NHS and local councils are working together to create a local version of this plan, along with a health and wellbeing strategy, to give people the best start in life, can live the life they want, and be supported in their choices when they need help. This will focus on improving people’s physical and mental health, and support them to stay well through every stage of their lives.

We have spent the past three years talking to people about what’s important to them, and what they’ve told us has been used to help shape our plans, and improve our health and care services

While we have made some significant improvements we need to do more to deliver our aim of delivering world-class care for major health problems, reducing health inequalities and supporting people to age well.

We face some big challenges, which will influence the future of health and care, including:

• Our population is expected to grow by more than 62,000 people in the next 20 years.
• People are living longer, often with multiple illnesses, such as cancer, heart problems and type 2 diabetes.
• Preventable illnesses like type 2 diabetes are increasing, and the amount of time people live in good health has been decreasing since 2012.
• Health inequalities and deprivation are increasing – people with severe and prolonged mental illness die on average 15 to 20 years earlier than those without.
• Vital health and care jobs remain unfilled, with an ageing workforce and a high turnover of care workers.
• We need to make better use of technology.
• There have been increases in NHS funding, but peoples’ needs for services are growing faster.

We’ve identified 12 areas that would help us to improve people’s health and wellbeing:

1. Improve maternity services 
2. Improve services for children and young people ( birth to 25 years)
3. Increase support for people to live a healthier life
4. Make care and support more personal
5. Transform care provided in the community that can keep people at home
6. Review pathways of care to improve outcomes for people with major health conditions
7. Improve how we respond in a crisis
8. Improve mental health services
9. Improve outcomes for people with learning disabilities and /or autism
10. Improve hospital services
11. Digitally enabled care and support
12. Improve how we make the most effective use of our resources.
